Employers offering health insurance are required to post certain notices and make them available to employees. For your convenience these notices are posted here. They are also available on your Paylocity Self-service portal under the "Benefits" tab. 

You can find a copy of our updated Creditable Coverage Notice, Exchange Notice, Notice of Availability of Privacy Practices, Privacy Practices Policy with Med-Pay, The WHCRA Enrollment Notice (Women's Health & Cancer Rights Act), and the CHIP Notice.
