1. Go to IHOPKC Benefits

 2. Click on the Dental & Vision Insurance tab

3. Click on either the Dental Plan link or the Vision Plan link

This will take you to the dental or vision site to learn more about each plan and the cost.

4. Click on the pop out summary page.

This will give you a breakdown of the plan and what it covers.

5. From here you can download, print or expand the summary pages to view.

To expand click the plus sign at the bottom of your screen.

6. Enroll by logging into your Paylocity self-service portal and navigate to the Benefits section. Follow the prompts to enroll.
