1. Sign in to your Paylocity account and go to your Pay section on your dashboard.

2. Click on More...

3. Click on Tax Forms

Click on Tax Forms

4. Select the type of tax form you want. Select the Tax Year needed.

5. To see a downloadable PDF, click on the View PDF link as shown below.

Follow the password directions, per your choosing. You can bypass the Password protect if you do not know your password.

To see a PDF for downloading, click on the View PDF link as shown below.
